This is the one occasion in the list whose classical rules run the opposite way to everything else, and that inversion is the clearest proof that muhurat rules are per-act rather than a general ranking of good and bad days. Surgery is a cutting, so the sharp nakshatras — Ardra, Ashlesha, Jyeshtha and Mula — which are barred for every beginning on this site, are the appropriate stars for it. Tuesday, avoided almost everywhere else because Mars brings conflict and blood, is the surgeon's weekday for precisely that reason. And the waning fortnight is preferred where the procedure removes something, because the Moon is decreasing; the waxing fortnight is preferred where tissue must heal and rebuild.
The rule that matters most is about the Moon's position relative to the body. The Kalapurusha scheme maps the twelve signs onto the body from head to feet — Aries the head, Taurus the face and throat, and so on down to Pisces the feet — and the classical instruction is not to operate on the part governed by the sign the Moon currently occupies, nor generally on the two adjacent to it. That check needs the specific body part, which a calendar cannot know, so the dates below apply the general tests only and leave the Kalapurusha check to be made against your actual procedure. Where a surgeon offers you a choice of dates, this narrows it. Where they do not, take the date they give you.

Frequently asked questions
Why are the "bad" nakshatras the good ones here?
Because the classification describes what a star is fit for, not whether it is fortunate. The Tikshna or Daruna group — Ardra, Ashlesha, Jyeshtha, Mula — governs cutting, separation and decisive intervention. That makes them wrong for a wedding and right for a scalpel. Reading them as simply inauspicious is the most common misunderstanding of the whole nakshatra system.
What is the Kalapurusha rule and why can this page not apply it?
It maps the zodiac onto the human body — Aries the head, Taurus the face and throat, Gemini the arms and shoulders, down to Pisces the feet — and advises against operating on the region ruled by the sign the transiting Moon occupies. Applying it requires knowing which part of the body is being operated on, which is information this page does not have. Check any shortlisted date against your own procedure before using it.
Should a muhurat ever change a surgery date?
Only when the surgeon has genuinely offered a choice and is indifferent between the options. Delaying a clinically indicated procedure for a calendar is not something this tradition asks of anyone, and no reading of the texts supports it. If the two ever appear to conflict, the medical judgement wins without further discussion.
